Submitting and reporting on incidents

You can record all site incidents in Zero Harm. Watch the video to show you how.

The type of Incident Report you’ll be required to submit will be dependent on the region and place that you live. Below we have collected a range of resources which will assist you when reporting a incident.

When you create an incident report as a manager you have the ability to assign labels to improve follow up work flows, categorise the incident or simply include extra detail to better filter your incident reports.

You can view individual and clustered incidents as spatially recorded through longitude and latitude positions.

Recording an incident

To create a new incident, click “New Incident”.

Zero Harm will automatically locate your site but you’ll have to zoom in and locate the position of where the incident occurred.

Fill out the details and create the incident.


For more information regarding if your incident is notifiable, click on help me to decide and review the full WorkSafe definitions.

Select the person who was affected – if any.


Please note that people can only be assigned to an incident if that person is associated with the site. This can include employees or people who’ve inducted or signed into the site.

 

Select their name from the list to proceed and write an optional description about what happened.


You can click “help me decide” to find out if the injury is considered serious.


If it is a serious incident it’s your legal responsibility to report it to Worksafe.

You now have a secure log of your incident. It is important to log all incidents on your site, regardless of if they need to be reported directly to Worksafe. Doing so establishes your site is taking appropriate measures to report incidents.

Returning to the incidents page we can see all incidents. These are saved in the cloud and can be accessed or shown to appropriate parties should the need arise.

Is this a reportable incident?

Although Zero Harm is not a regulator and cannot advise about reporting a work place incident, you can contact your state, territory or safety authority for further details here;

When you create an incident report as a manager you have the ability to assign labels to improve follow up work flows, categorise the incident or simply include extra detail to better filter your incident reports.

Labels help you to filter and report on incidents. You can use labels for status/workflow, injuries, incident types, and root causes.

Viewing incidents on maps

Here you will be able to view individual and clustered incidents as spatially recorded through longitude and latitude positions.

 

To view the locations of your work site incidents from an overhead satellite map perspective click on Incidents from the Site Administration menu and then Map via the split page menu.
